OFFICIAL RULES

NO PURCHASE NECESSARY TO ENTER OR WIN. MAKING A PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. The Curbell Plastics DIGITAL CUSTOMER SERVICE EXPERIENCE SURVEY SWEEPSTAKES (the "Sweepstakes") is sponsored by Curbell Plastics, Inc., 7 Cobham Drive, Orchard Park, New York 14127 ("Curbell"). The Sweepstakes is open to U.S. companies located in any of the 50 United States, including the District of Columbia.

1. HOW TO ENTER: Enter your email address after completing the Digital Customer Service Experience Survey to register for the drawing. By entering, you certify that your company is eligible to participate. Entry period is December 19, 2023 – January 19, 2024. Limit one entry per person for the duration of the Entry Period. All eligible entries received are eligible for the random prize drawings. Normal time, toll, connection and usage rates, if any, charged by your Internet service provider will apply. Automated, mechanical, or third party entry devices or services are prohibited.

2. RANDOM DRAWING: Three (3) winning participants will be selected in a random drawing on January 22, 2024 from among all eligible entries received during the entry period. "Eligible Entries" are those entries received by Curbell throughout the Entry Period, which meet all eligibility requirements set forth in these Official Rules. Random drawing will be conducted by Curbell whose decisions are final and binding with respect to all matters related to this Sweepstakes. Winner will be notified by email, phone, or US mail. If winner cannot be notified or is unwilling or unable to accept the prize, an alternate winner will be selected in a random drawing. Odds of winning will depend upon the number of eligible entries received.

3. PRIZE: Subject to verification and compliance with these Official Rules, three (3) participants that are the REWARD Winners will receive one (1) $100 Amazon Gift Card (approximate value $100.00)(“REWARD”). Curbell may replace prize with items of equal or greater value in the event stated prize is unavailable. Prize is nontransferable, non-substitutable and non-endorsable.

4. GENERAL RULES: Open only to U.S. companies located in any of the 50 United States and the District of Columbia. Void in Puerto Rico. Employees of Curbell, Inc., its affiliates, subsidiaries, divisions, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible to participate or win the REWARD. Void where prohibited by law. No substitution or transfer of prize by winner is permitted. All federal, state and local taxes, and any other costs associated with the prize, are the sole responsibility of winner. All federal, state and local laws apply. All entries submitted become the sole property of Curbell and will not be acknowledged or returned. Potential winner may be required to sign and return an Affidavit of Eligibility/Release of Liability/Prize acceptance form within 7 days of attempted notification. Noncompliance within this time period or return of any prize/prize notification may result in disqualification and an alternate potential winner will be selected in a random drawing from the remaining eligible entries received. Unless otherwise prohibited by applicable law, acceptance of prize constitutes permission to Curbell and its agencies to use winner's name, city, state, and statements made by winner for purposes of advertising, promotion and publicity in any and all media, now or hereafter known, throughout the world in perpetuity without additional compensation. By accepting prize, winner agrees to hold Curbell, its affiliates, subsidiaries and divisions and the respective directors, officers, employees and assigns of each of the foregoing, harmless against any and all claims and liability arising out of acceptance and/or use of prize. Winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in this Sweepstakes or use or redemption of any prize. By entering this Sweepstakes, entrant acknowledges and affirms that participation in this Sweepstakes does not violate the entrant’s employer’s Code of Conduct or any similar employer policy or rule and waives all rights to claim punitive, incidental and consequential damages, attorneys' fees or any damages other than actual out-of-pocket costs incurred to enter. By participating in this Sweepstakes entrants agree to be bound by these Official Rules and the decisions of the judges. Curbell is not responsible for incomplete, lost, late, damaged, illegible, misdirected entries, for technical hardware, software malfunctions or failures of any kind, lost, unavailable network connections, or failed, incomplete, garbled or delayed computer transmission, which may limit a user's ability to participate. Curbell reserves the right in its sole discretion, to cancel, suspend or modify this Sweepstakes (or any portion thereof), or to disqualify any entrant(s), if any fraud, virus, actions by entrants, technical or other error or problem, or any other occurrence corrupts or affects the administration, integrity, security, or proper play of this Sweepstakes, as determined by Curbell in its sole discretion. In the event of cancellation, Curbell reserves the right, at its sole discretion, to award the prize in a random drawing from among all eligible entries received up to the time of the event or action warranting such cancellation. In the event this Sweepstakes is cancelled prior to the stated end date, a notice will be posted at www.curbellplastics.com. CAUTION: Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of this Sweepstakes may constitute a violation of criminal and civil laws, and should such an attempt be made, Curbell reserves the right to seek damages from any such entrant to the fullest extent permitted by law, including criminal prosecution. In the event of such a dispute or in the event a dispute arises regarding the specific company entitled to receive the prize, entries will be declared made by the authorized account holder of the email address submitted at the time of entry and any damage made to the website will also be the responsibility of the authorized account holder. "Authorized account holder" is defined as the company that is assigned to an email address by an Internet access provider, online service provider or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. Curbell is not responsible for any typographical or other error in the printing of the offer, administration of this Sweepstakes or in the announcement of the prize or for any liability for damage to any computer system resulting from participating in, or accessing or downloading information in connection with, this Sweepstakes.

5. COPY OF OFFICIAL RULES; WINNERS LIST: For a copy of these Official Rules, or for the name of the winners, send a self-addressed, stamped envelope to Curbell Plastics, Inc., 7 Cobham Drive, Orchard Park, New York 14127, Attention: Marketing. Requests must be received by January 19, 2024. (Vermont residents may omit return postage).

7. NO WARRANTIES: Curbell makes no warranties covering the prize furnished as part of this Sweepstakes.

8. GOVERNING LAW: This Sweepstakes and the rights and obligations of Curbell and participants will be governed by and controlled by the laws of the State of New York, applicable to contracts made and performed therein without reference to the applicable choice of law provisions. All actions, proceedings, or litigation relating hereto will be instituted and prosecuted solely within the State of New York, County of Erie. The parties consent to the jurisdiction of the state court of New York and federal court located within such state and county with respect to any action, dispute, or other matter pertaining to or arising out of this Sweepstakes.
